Это пятая часть серии игр Wonder Boy. В Monster World всё было спокойно и все жили в мире, но однажды этот покой нарушили монстры во главе со зловещим BioMeka. Главный герой игры Шион (Shion), поклялся во, чтобы это не стало очистить земли Monster World от монстров и раз и навсегда покончить со злом. Данная часть игры претерпела значительные изменения по сравнению с предыдущими частями. Теперь в игру были добавлены элементы РПГ.
